EQUIPP® Account and Log In Information

1. How to obtain EQUIPP® Access:

- We have contracts with several Pharmacy Services Administration Organizations (PSAO) and third-party contracting groups. In some cases, pharmacies will need to go through their specific group to register.
- Pharmacies that are not affiliated with an organization can obtain access through an independent contract by contacting EQUIPP® Support at support@pharmacyquality.com
- How to log into EQUIPP®:
- There are two primary ways to access EQUIPP®:
- Directly logging into EQUIPP.pharmacyquality.com – Users are sent an email with login directions after their new account has been created. The welcome email contains a link that will prompt users to set their initial password and set up Multi-Factor Authentication (MFA). A step-by-step guide to Multi-Factor Authentication (MFA) can be found here. If you have not received a welcome email, please click on the "forgot password" link located on the EQUIPP login page. If your username exists within EQUIPP, you will receive a password reset email from support@pharmacyquality.com. If you DO NOT receive an email, please reach out to us directly at support@pharmacyquality.com for further login assistance.
- Single Sign On (SSO) Login – Some pharmacy organizations require pharmacies to log in through their site via SSO. Those with an SSO login will not need an additional EQUIPP password to log in. If your pharmacy organization utilizes an SSO, please log in through the designated channels and contact EQUIPP® Support if you have any difficulties logging in.
- There are two primary ways to access EQUIPP®:
- How to update account information:
- Name Updates: End users can update the first and last name associated with their account through the Settings tab. The Settings tab can be found by clicking on the orange circle icon at the top right of the Performance Dashboard. (See image below)

- A pop-up window will display with the option to enter the new First and Last Names
- Click 'Confirm'
- This will save your changes and display them within the EQUIPP Dashboard

- To add or remove Pharmacies from your EQUIPP Dashboard: Pharmacies should place all requests to add or remove locations through their PSAO.
- To update usernames or add additional users: Pharmacies will need to place all account updates and new user requests through their PSAO

- How to reset your password:
- End users that log in directly into the EQUIPP® site should set their password upon first login with the "Set a password" link provided in their welcome email. After the initial login, end users will be able to reset their password with the “forgot password” link.
- If you see an error message when logging into EQUIPP®? (Error message, “Getting Data” loading screen)
- We recommend that you try each of the following steps:
- Clear the browser cache and cookies
- Log in through a different browser
- Log in through a different device
- Pause the browser ad-blocker (if applicable)
- We recommend that you try each of the following steps:
- If you see an error message that states "No Associated Groups Found"?
- This can populate if you transition from one PSAO to another
- If a PSAO updates registration information and the username is no longer registered to a pharmacy
- If your account is deactivated from leaving one location and going to another

If you need to reset or change the phone number attached to the MFA security feature, please see the instructions below:
- Once logged in successfully, click on the orange circle located in the top right portion of your EQUIPP Dashboard. (See Image Below)
- Click 'Settings'
- A pop-up window will display with an option to change either the first and last name on the account, or 'Reset MFA'.
- Click 'Reset MFA'
- Another pop-up window will show confirming that you will be logged out and then be prompted to re-establish the newly desired phone number to be utilized in the MFA process.
